Richard, D'oh! I forgot that in the interval since I wrote my book I migrated from tetex to TeXLive. Sigh. However, copying it to /usr/share/texmf-dist/tex/latex/base/, reconfiguring lyx, and restarting the application did not resolve the error. I'll read the latest users guide and see what it tells me.
