Well I never had an iPhone 4, so perhaps the hardware addressed this issue last year. I can tell you that on my 3GS, the other party almost always heard the call waiting signal, and more over heard who was calling, not to mention text messages. This was even if I attached the headphones and the mic was far from the ear buds. On the 4S, the other party can't seem to hear voiceOver even though the output volume would normally be sufficient for the speaker to pick it up.
