Aa, so you do it from the receipt, not from within ITunes, nor by calling Apple directly. Maybe that's where I went wrong.
Wonder if apps not being accessible with Voiceover has yet been worked into their policy as a valid excuse for getting a refund, provided you don't obviously make a habbit of doing it, I spoke to someone in Cupertino who's name will be left unsaid for curticy sake for them, but anyway, he or she told me very very directly that they were going to start working on getting that in their list for valid refunds. That would be good if they do.
