I've not found sound to be muffled/smothered in any way. That's just me though. Others may have a totally different opinion.

Normally the microphone isn't smothered because its to one side of the dock connector but I take your point, if the phone is in a horizontal position then its most likely that the microphone will be exposed to more sound.
