Hi Sarah, Some obvious questions: Did you repair permissions after installing your software updates? This only restores the permissions of your native Apple software to the expected defaults, but in most cases it will also fix other software issues. If you're using an IPA keyboard, then I'm guessing this is a third party installation -- probably the one from SIL.
Here are a few thoughts: you might have corrupted preference (plist) files. The ones associated with language and keyboard selection are named com.apple.HIToolbox.plist (stands for "Human Interface Toolbox"). Do you have a secondary test account on your Mac you can log into and check whether this bug only affects your main account? If so, it's likely that it is your individual plist files under you account that are the problem. What happens if you uncheck the IPA keyboard as one of your input keyboards, so you only have U.S. English? If you restart your Mac, I assume you should still get your default U.S. English input. Then, if you add the IPA keyboard back, does this behave correctly? The Command-Space shortcut for switching input keyboards used to conflict with the shortcut for Spotlight searches, so we used to have to check the default shortcut assignment for switching language keyboards under System Preferences > Keyboard > Keyboard Shortcuts > Keyboard & Text Input (I think these are renamed in Mavericks so the last two are Shortcuts > Keyboard). Anyway, the shortcuts you want to check are for "Select the previous input source" and "Select next input source". We used to have to fix these shortcuts by hand. Perhaps start looking there to find out whether there's a conflict? Sometimes you can just try moving in versions of older plist files from before your update. Because these are small files, I'll periodically keep copies of the most recently modified 50 or so preference files under user's preference library. (Sort these in list view under Finder according to modification date). That way, if I suspect there's a recently corrupted plist file, I can swap in a version from when things were working without even having to look at a system backup. HTH.
