Do I press command-r while the Mac is running and it reboots, or do I
shut it down and then boot while holding command-r, ... or some other how?

You'd need to boot into it with command-r, and then after turning
on VoiceOver, select, Terminal, from the Utilities menu. You would
type, resetpassword as one word, and from that dialog, you can
reset all of your permissions in one shot.
