Keyboard Layout Canada CSA default?

The keyboard layout on my computer keeps reverting back to Canada CSA - 
which I do not want.  I change it to U.S. and somehow within days, 
sometimes hours it reverts back to Canada CSA without my noticing how or 
when.  A couple of times this has caused real problems when I did notice 
it when filling out forms which were then submitted.

Something must be going on here.  Does anyone have any idea how to 
select the U.S. keyboard layout and have it "stick".  Could some sort of 
third party software I have installed or system setting I have chosen 
automatically change this setting?

The computer was purchased from a Canadian reseller, as was OSX 10.2.
