You can do this in (of all things) Preview, at least under Leopard. Select 'Extract Shape' from under the Select item in the toolbar and drag around the horse.
OR... if you want the image to use over another picture, you can use the 'Instant Alpha' item from the same toolbar item.
I cannot say if this is a good solution for printing, but I've used it with success on the screen.
