I use an Index printer and Index has an application and driver. I use liblouis and liblouisutdml for translation but I also do a lot of editing along with that; there is also a louis program for the Mac but at least for the Spanish there were problems with it last time I tried it but the English may work fine. I'm just doing text files though, not Word files, but I don't need that for what I am doing right now anyway. If you have an Index printer, a quick google should bring up information as this was posted on the macvoiceover list and I think some other lists a little over a year ago.
