Hi,

It depends on what you're going to use the form for and how you like the input 
to be reported back to you.  If it's something like a survey or questionnaire 
where you don't mind the results being returned in a spreadsheet, then a Google 
Form is my best suggestion.  If you have a Gmail account, then you have access 
to Google Forms as part of the Google Apps suite.  I thought I read somewhere 
that you can use Google Forms in another fashion as well where the form and 
response are kept together but I haven't figured out how to do that yet, or 
maybe I'm just mistaken.
