Hi Gigi, I've tried doing this before. PDFPen Pro did a pretty good job inserting the form fields, but someone with vision told me that some of the fields to type the answers were in the wrong spots/not aligned properly. They were able to click and drag and stuff to fix it up a bit, but this would not be possible as a VoiceOver user. The other problem is that all the form fields were unlabelled, so I sometimes couldn't tell what answer I should be writing in which field.
