Good News!

I finally got it resolved. I had two users set up on my Mac, and decided to go 
back to the one I rarely use. That user did not have the problem with the 
disappearing Extras menu, so I started to compare things between each. A lot of 
back/forth that I will not bore you with.
The problem was in the System Preferences under General, which I rarely look 
at. But this morning when trying to fix something else, I found the checkbox 
for “Automatically Hide and Show the Menu Bar”. This was checked, and after 
unchecking it, the problem was resolved.
