Eric, this sounds like a faulty usb connector to me.

I and a friend had identical thumb drives and I used both on my Mac. Mine would 
consistently be recognized and stay that way until I ejected it. However, his 
would not. It would either not be recognized, or be prematurely disconnected 
with no warning at all, which would bring up the exact same dialogue you 
mention.

The problem with his drive got worse and worse until it was unusable on either 
a Mac or Windows system, so this might be something to check into…
