Okay.  Were I in your situation, and I had all that happening, I'd go for a 
disk repair first.  You can effect this by turning off your machine, then, when 
you turn it back on, make sure you hold down the command+r keys.  This will 
cause the run time OS to start up.  You want to wait about say 45 seconds or so 
then press comand+F5 to start voice Over.  You will note that Fred will be the 
voice you have here.  You will notice a menu of things from which you can 
choose.  Pick the unilities option and from there, I'd select to repair the 
Macintosh drive.  IF even repaiaring disk or disk permitions fails, then, I'd 
be a little more dramatic and go for a fresh installation of the OS.  Very easy 
to do using Lion.  Some here say that a fresh install won't make much of a 
difference;  however, as I read your description of your  probem and imagineing 
what I myself would do, those would be the things I'd try.
