FWIW I use
user ALL = (ALL) NOPASSWD: ALL
root ALL = (ALL) NOPASSWD: ALL
because I like to do things like 'sudo vi /etc/somesystemfile'.

That's because you are running passwd as user, and user doesn't have a
password set. If you run 'passwd user' as root, it should work.
