It could be that you have removed the USB cable without 'ejecting' it first from Windows (if you use Windows). It can be done from the icon on the system tray. In N770 the trick for safe MMC usage is to wait for few seconds after opening the memory card cover, in case the warning dialog comes late or unmounting takes longer than usually (e.g. because of low RAM).
