There is a long standing issue with gmail <https://wiki.list.org/x/4030680>. Gmail discards the list's message as a duplicate of one in the user's sent folder, but I'm not aware of this affecting outlook users.

I thought it was the 'nodupes' setting so I turned it off for everyone on the list.

It's not. nodupes controls whether a user addressed directly in To: or Cc: receives the list copy.

Every member has 'hide' on, and 'ack', 'not metoo', 'nodups' and 'digest' turned off

Is it Outlook?

Yes. It might be something in her outlook settings or outlook itself.

Is it Mailman?

No.
