Yeah, it can be used as such. Check out mod_tile, tirex, paleoserver (WMS), 
and friends.

Not really. I've used huge DEMs with no problem at all. Just store all your 
imagery correctly and you'll be fine. Having imagery as PostGIS raster can 
prove useful.

I wouldn't worry about map size limitations - 2000x2000 maps render fine. If 
you need to do heavier raster processing with Mapnik, then you're using the 
wrong tool. Resort to GRASS or something raster-specific.

Use overlays!

Don't render a new image with new layers off/on. Render the layers as separate 
overlays, make client-side software put them on top of each other, and switch 
them on and off.
