I just read the report.

1. This seems to be the same scenario as the Charleston SC fire - roof
not vented - front windows busted - fire blows up

2. If E30 had 4 people and L25 had 4 people - L25 would send 2 to the
roof and 2 inside - leaving no one at the turntable I suppose - Engine
25 would have 1 person on the pump - maybe 1 person at the hydrant
initially - and 2 inside.

3. Newton Eng 10, Brookline Eng 6, Dedham HQ Eng, and dedham HQ Ladder
all were close to the scene - but apparently not called at all. (That
would be SOP - Boston only starts to get mutual aid on the 5th alarm
IIRC - and that is for coverage only).

4. ICS was not used. So?

5. Rescue 2 couldn't function as a RIT for several minutes. Really?

6. Back to Basics Classes - dont they train daily in the station on
that basic stuff?

7. It seems strange that there was a "rapid fire event", but the fire
seemed to go out immediately.

8. If there were 2 RIT teams standing at the front door and the back
door, both with a charged 2.5 inch line, and a thermal camera, maybe
they could have charged in and saved the 2 guys.  Or maybe both guys
had fatal brain injuries. (But the cause of death was listed as heat
and smoke). (Were the victims found in burning debris or burning
grease? - I dont remember that being mentioned).

9. No mention of any hydrant or pumper problems.

I have to go back and read the NIOSH report and the BOI report again.
