Looking at the BOI Report - 1. That might have been a classic "smoke explosion". An ignition of carbon monoxide with a pressure wave. Did the fire blow out the front window or was the window already busted open. Maybe it was a good thing the window was already vented - the pressure wave went mostly outside instead of dropping the roof.
2. Fire damage in the kitchen was low at the floor level. The chairs and table by the street window ere charred. 3. The portable radio was on Ch 1 instead of Ch 2 - not that it made any difference in this case. Maybe it did. Maybe the Ch 1 traffic was a distraction. 4. L25 went inside with 3 people. E30 had 2 people outside hooking up to the hydrant. 5. Running card shows no mutual aid to the fire. 6. Did taking out the window cause the smoke explosion? The timing indicates it might have. The BOI thinks that oxygen in the water stream contributed to the rapid fire event. Ineteresting. 7. Thought just occurred to me - how about getting some paramedics trained to use SCBA so they can be part of the RIT? Lets say the victim is tangled in debris in a smoky area, but the fire is knocked down (or at bay). If a paramedic could swoop in and insert a airtube down the throat - and start feeding O2 or air to the victim - and doing CPR - or apply a AED. If none of that is feasible - then rapid removal from the dangerous location is obviously the only hope. 8. Maybe the SCBA mask gave a false sense of security in this case. If a fireball goes off in your face, you have maybe 1 or 2 seconds to react. The margins of safety are very slim at that point. (Obviously - I know - just stating the obvious to cover all the bases). 9.
