Unfortunately, during the day yesterday one participant did not realize that the first row was a header row and the column headers got overwritten. I have restored the headers and added a note to indicate that a participant should scroll to the first empty row to enter their information. My apologies to those who entered their information without the benefit of the column headings- you did a great job figuring out the questions on your own.
