I've seen little tiny pin-prick size "holes" in the silver colored coating of my
CDs and they've never caused any problems (AFAIK).  I guess the same
thing might happen to MDs?

Or did you use the test mode and turn up the laser power to "level 17"
(sufficient to vaporise a baseball sized lump of Tungsten in 2 seconds).

One possibility is the nanites present in all MD equipment which keep
it functional are attacking the disc.  Or something like that.
