We have recently applied for an application for a Consumer
Use Tax Account from the State Board of Equalization. SBE has requested
that someone on OCFA's Board sign the application and submit a copy of
his/her driver's license and social security card. OCFA's Board
Chairperson signed the application, but he is not comfortable providing a
driver's license or social security card.
Do you recall how you handled the personal
information requirement for the SBE Consumer Use Tax application? Thank
