I personally never discount operator error, since I've done more
than a few knuckle-headed things in my life.
I would check to make sure there is no possibility that you are
drawing air into the pump while charging. Sounds strange, but if you
had a leak somewhere that allows the pump to inject air along with
the freon, guess what?
