You will probably need to take the rear suspension apart to properly
diagnose it, my bet is on tired springs and tired rubber (subframe
bushings, spring pads, differential mount).  Additionally could be a broken
spring or two.  Rear lower control arms can rust and collapse too.  This is
a big job (removing the entire rear axle assembly), approach with caution.
You probably can get away with just the springs, pads, and the two front
bushings (not lower control arm bushings but the front two "horns" of the
carrier, located underneath and just below and behind the rear doors).
Differential mount is also fairly easy.

Read up in the factory service manual, there are different spring and
spring pad combinations depending on optional equipment installed at the
factory (i.e. automatic transmission, air conditioning, sunroof).

The front end / tire wear problem is not related.
