A copy of the NADA or KBB also works (if it shows favorable numbers).

Revenue) form?

Buyer and seller both sign it.  That sets the value of the car to reality,
for tax purposes.  They will also accept a signed estimate of value from a
recognized dealer, on letterhead.

WA is one of the few States with no income tax.  They make it up, elsewhere.
