   It seems that you may have one, or more, of a few problems. 
First, potential culprit, is the type of serial device you are
using.  There are occasional problems with the Amiga's native
serial.device.  I am using artser.device, as a replacement de-
vice (found on Aminet).  Next, you do have to, even, do a cold
boot, at times, because something is not being cleared from RAM.

   Second, if all of your settings are correct, for your ISP and
your modem and your telephone service (tone vs. rotary/pulse),
then, the culprit can still be the phone company or the ISP's
servers.

   Before I, or anyone else, can hazard a real plan of action,
for you, we need more info about your configuraation of Miami
and your computer, etc.
