It might help to show pf.conf then. If you're allowing it to create state on intermediate packets (usually by accidentally having packets passed by the implicit "pass flags any no state" rule which applies if there's no other matching rule), you can run into problems with TCP window scaling which might give the results you're seeing. The other bits you posted don't suggest any problems on the physical side.
