FWIW, I have my workstations behind two (i.e. in series) firewalls/routers (PC engines APU1) running OpenBSD, and have had a couple of MS Teams meetings happening along with some video streaming, without any issues. Nothing special on the pf.confs, other than prioritizing TCP ACKs. Do see the issues you describe on more than one machine? Do they run OpenBSD, or other OSs? Do any of the counters on `netstat -s` hint at something? --
