Per Your Request (Thanks again Vivek):

According to -MCPAN -e shell I *am* using the most
current Apache::Sandwich.

For those of us who have just tuned in, I am installing
mod_perl1.21 on redhat6.1 apache1.3.9 and generally
using the CPAN shell and have installed just the basics
(MD5, Bundle::libnet, Bundle::Apache, Bundle::CPAN,
 Bundle::DBD::mysql)

The perl Makefile.PL AND make of mod_perl
report nothing unusual.
make test groans...

modules/sandwich....FAILED test 1
        Failed 1/1 tests, 0.00% okay

Sadly make test TEST_VERBOSE=1 doesn't provide any
more insight with...

modules/sandwich....module Apache::Sandwich is installed
1..1
not ok 1
FAILED test 1
        Failed 1/1 tests, 0.00% okay
