Code reorganization to reduce number of modules linked into unit tester (3/n)
Move all the migration code out of revision.cc to a new file
migrate_ancestry.cc (except a small piece to paths.cc instead).
Rename *_migration.cc to migrate_*.cc for consistency. Rename
schema_migration.hh to migration.hh and put the prototypes for
migrate_ancestry.cc here too.
