The policy is simply to increment the version number after each upload,
see [1].  If several maintainers make changes in the same upload,
separate the changelog entries thus:

monotone (1.0-18) unstable; urgency=low

  [Maintainer A]
  * change 1
  * change 2.  Closes #12.

  [Maintainer B]
  * change 3.

 -- Maintainer A (or B), whoever makes the last change -- date
