In response to the fluency question, I was quite excited by the response to 
look for root cause.  At the high school level, I know we look at that 4th 
grade reading level as a major benchmark.  Students who have average or above 
average intelligence are frequently able to sight read up to the fourth grade 
level.  However, if they don't have sound-symbol correspondance and phonics 
down, it is very difficult to read efficiently above the fourth grade level.  
Have you looked at these skills?  As a 5 minute quick assessment, put the 
vowels and vowel teams on flash cards.  Ask the student to identify the sounds 
each vowel or vowel team makes.  If this is a struggle, it might be a good idea 
to complete a full assessment in this area and design/use an intervention 
surrounding letter sounds and syllable types.
