Soundex is bombing when you try to pass it a Null last name.  Change the
code to:

  Set rst = CurrentDb.OpenRecordset("SELECT LastName, FirstName FROM " & _
    "client WHERE Soundex(NZ([LastName], ' ')) = '" & _
    Soundex(Me.LastName) & "'") 

You also have a problem that the form always thinks it's "dirty", so you get
an error on the required SSN number when you try to clear and close the
form.
