Following some further testing, this is feeling more like a kernel issue than a 
PCSCD issue :( 

I got up to 10 readers fully registered with PCSCD. However I then started 
getting: 
Mar 24 10:24:16 netisam kernel: xhci_hcd 0000:00:14.0: Not enough bandwidth on 
HS bus for newly activated TT.
Mar 24 10:24:16 netisam kernel: xhci_hcd 0000:00:14.0: Not enough bandwidth

Device would enumerate with lsusb, but no record of reader in pcscd. 

And a few readers later I got: 
Mar 24 10:24:26 netisam kernel: xhci_hcd 0000:00:14.0: Not enough host 
resources, active endpoint contexts = 63

There was then no record of the device in lsusb or pcscd :( 

Anyone familiar with any of the above errors?
Or anyone able to point me in the right direction for support on xhci_hcd?

Cheers for the suggestion. I've tried the latest CCID drivers (1.4.16) and 
don't appear to be experiencing the same stability issues. Am now up-to 30+ 
plug/unplugs without any USB errors... 

Now to try with a few more readers...

If you use a proprietary driver you should talk to the vendor.

The reader should be well supported by the free software CCID driver, so better 
try that one instead:
