Just the header and the footer is muse based. Essentially, bibtex2html is rich enough to put URLs in, while bibtex ignores them. This is all actually quite hard work. I have to use perl to hack the bibtex2html produced documentation to get the URLs right, and it's tied together with some baroque make files. I'm happy to show you stuff if you'd be more interested.
