i think that UK countries should indeed be included. i would prefer to
see "England, UK" rather than "UK" or "England".

"New York, NY" "City of New York, NY" and "New York City, NY" are
interchangeable AFAIC. I think any use is fine, but if there needs to be
a solid rule about this, i feel that it should be "New York, NY, USA"
