I've not been interested in it much myself, but as best I can recall
from what I saw on the list, S/MIME would be trivial to do, might
not even require any mods to mutt itself, just config files, if only
there were a helper program, after the style of pgp or gpg, to do
the actual work.
And we're waiting for someone to step up with such a helper program.
I recall rumours that some of the hard parts of the job might be
handled already by some of the less-well-documented nether reaches
of OpenSSL. But nothing sufficiently detailed to start testing
with:-).
