At least in the version of pdftotext shipped with Red Hat 6.2
(0.90), pdftotext(1) claims that if the output file is "-", the
output will go to stdout. In which case you shouldn't need your
wrapper, just put
application/pdf; pdftotext %s -; copiousoutput
in your mailcap.
