I just spotted a thread in some archive that discussed this.  The 
capturecard IDs have gone out of range (16?) with all the changes you've 
made, doubtless deleting and re-adding capture cards.

If you can reset the capturecard IDs in the database to low numbers less 
than 16, your live TV should work again.

I used phpmysql to tweak the IDs, but the backend wasn't happy with the 
results.  I then deleted and re-added my capture cards once more, and 
the ids are now 1 and 3, and I have live TV again.

Someone more adept at modifying the table may chime in here with a more 
elegant solution. :-)
