Maybe even PHP-GTK (Win or *nix)

There is also the Agata project that is a PHP-GTK frontend to Posgresql, I
think someone is working on a MySQL version.

PHP-GTK supports libglade allowing you to use Glade
http://glade.gnome.org/ generated GUI.

I think most of these are fairly new, it may not suit what you are after.
