I have been getting the same error every so often. It is only on a machine that does about 900 queries per second average. I am running a 2.4.20 kernel with MySQL 4.0.14. This only happens with MYISAM tables and only ones with high INSERTs and SELECTs. It seemed to get better after increasing the max open tables to 512. I don't have much more info than that unfortunately.
