Hi Matt, I'm sorry to say that even after upgrading to MySQL-4.0.16, our server is still suffering from the exact same symptoms, albeit on a Linux system.
We have recently done some serious testing to rule out hardware failure, but the error persists. Tables corrupt and mysqld restarts with a signal 11 error code approximately every tenth minute.
