Portage has generally done a good job of keeping up with released versions. 
For more bleeding-edge stuff (like ivtv 0.3.x or MythTV CVS), there are
ebuilds you can install in $PORTDIR_OVERLAY so that you can still have
Portage manage everything.  Everything on my MythTV box was installed
through Portage.
