I don't know how much of a solution this is, but you
might try hitting the "m" and "n" keys while playing
in kaffeine.  This is supposed to adjust the
audio/video sync by an offset in xine.  Of course, I'm
assuming this will work in kaffeine, which is supposed
to be based on xine, so the keys might be different. 
And, this doesn't help with your standalone stuff.  Eh
well.

Sometimes I just transcode stuff myself from the .nuv
files in /mnt/store.  Programs like transcode and
ffmpeg allow for an audio/video offset to be added.
