One minor tweak to Mike's (excellently comprehensive!) instructions:

If your code is relatively self-contained (e.g. just a couple of code
files), and you don't wish to provide a separate copyright notice,
documentation, etc...
   then it's fine to just attach the basic code files, without
necessarily packaging them up into a tarball.    But if there
are a significant number of files involved, then a single bundle
is probably sensible.

Not if you keep faffing about and don't upload the files, no!   :-)

We are likely to start gearing up for the 5.2 release in the next
couple of weeks or so.   At which point we'll enter a code freeze,
and won't be accepting any new features until that's out.   So if
you want this to be included in the next release, then please upload
the files ASAP.

The longer you delay, the less time we'll have to consider this
code, and the less likely it will end up being included.
