The gateway is a NAT router -- it will exhibit the same symptoms as a
firewall ( audio/video out only) unless it has H.323 support -- check with
the supplier to see if the device in question can be so configured.

And yes no one will be able to call you -- consequence of the NAT with no
chance of it changing I am afraid.
