Could be a direct sound screw up in the sound drivers, could be faulty
drivers, could be a faulty sound card ( maybe even the design). It is also
possible that the audio mixer is (mis)configured to direct the wave output
through the record mix.

I have heard of this problem before and thought that a faulty headset was
the problem but it seems that you have eliminated that. What type of sound
card is this?

I think many of us would be interested in your diagnosis and resolution to
this problem so if you find anything keep us up to date.
