Both these are likely a function of lack of bandwidth --make sure your video
is the smallest possible both ways-- if you pause all video you may improve
audio -- if you set bandwidth settings lower you make more usable audio --
if you eliminated bandwidth sucking echo you may get more bandwidth for
audio and video

Echo you hear is caused by the setup at the other end -- the other end ( and
you ) must use headphones or headsets to get acceptable audio.

It depends on end to end bandwidth - if your or the other parties isp are
oversubscribed or poorly connected or the path between you goes through a
busy router somewhere packets will be dropped resulting in poor audio and
video.
