The "extensions" I was talking about were just changes in the
"Intercept" method of the LazyInitializer.  I'm using the new
NHibernate code now.  I basically just copied and pasted three small
classes, made some quick edits, and was on my way without any
dependency on nhibernate.bytecode.castle anymore.  Exciting!

It's quite enjoyable to see the reduction in deployment complexity
occurring lately.  Now we just need to move to .NET 4 with ISet and
get rid of the pesky Iesi.Collections. ;)  Don't hurt me!
